U SPORTS Central (Mandatory for all varsity athletes)
- Register and sign-in HERE
- First year student-athletes must REGISTER with U SPORTS for the first time and send their U SPORTS ID to their coach (Registration comes with a $50 fee which cannot be covered by the university through scholarship)
- All student-athletes, once registered, must sign-in and complete the U SPORTS Acknowledgment & Consent Form prior to competing for their respective team
- You can also update your athlete information including, jersey number, height (5'12"), weight (lbs), hometown, full legal name, academic program, and previous teams. Please use appropriate professional formatting when inputting this information. Ex: capital letters where they are needed
- If you have forgotten your USPORTS ID your coach has the list.

CCES Online Education now Sport Integrity Canada (Mandatory for all athletes)
- New student-athletes and staff are now able to create accounts using the below PDF invitation.
- Returning student-athletes and staff will log into their existing Sport Integrity Canada education accounts at to complete their 2026-27 e-learning course.
- The name of the anti-doping series has changed from Clean Sport to Anti-Doping

Safe Sport (Mandatory for all varsity athletes)
- Head to the following link and take these steps to complete the online training (estimated 90min):
- All participants, including student-athletes are required to complete the Coaches Association of Canada Safe Sport training module.
Please see the detailed step by step guide on how to start the process or creating a Coach.ca account, and complete your training. Once completed, send a screenshot of you completion to alicia.hurley@ufv.ca. Be sure to include your full name and which team you are on in your email. Completion of this training is mandatory to be eligible to play.
Medical Information (Mandatory for all athletes)
- You must go to the UFV Student Health and Wellness site and complete the following forms before you are able to participate with your team. Your medical forms will reset on May 1 of each year and you will need to go in after this date to be compliant.
- Incoming Athletes:
- Incoming athletes must complete the following forms
- Athlete Medial History
- Athlete Insurance Information
- Athlete Treatment Consent and Release
- New Athlete Physical Exam
- This is an external medical examination completed by your physician. You can download this form on the portal and bring to your physical exam for review and stamp to verify completion.
- Complete the Concussion Awareness Training Tool (CATT) and upload proof of completion to your student health and wellness account.
- Returning Athletes:
- Returning athletes must update their student health and Wellness medical forms annually. The following forms will need to be completed.

- Athletic Therapy appointments will be booked through the UFV Student Health and Wellness portal.If you are a varsity athlete looking to schedule an appointment, please ensure all forms have been completed. When selecting the treatment type, please see the following options:
- All injuries select "AT injury/illness visit"
- For a new concussion select "Concussion Assessment"
- For follow-up treatment regarding an existing concussion select "Concussion follow-up"
Facility Access (Mandatory for all athletes)

- If you have previously created an account, please do not create a new account. Follow the steps for a forgotten password.

- Once you have created an account you will need to come in person to the client services desk in Building E (E150), with your campus card, to sign your facility waiver , and activate your account before you will be able to participate.

Sports Information Form (Mandatory for all athletes)
- All athletes must complete the media consent waiver HERE to consent to photos and videos being captured.
- Incoming athletes must fill out the online form HERE if you have not previously completed it for a recruit release. Returning Athletes ,and athletes who have filled this out for a recruiting release, do not need to fill out this form.
